Remnant of Ner'zhul, Healer Guide
Updated 10 months ago for patch 9.2.7 by Preston. View the change log.
This page covers a healer-focused strategy for Remnant of Ner'zhul in Sanctum of Domination in Shadowlands. While it's tailored for healers, other roles may also find the information useful. If you have any suggestions or feedback, you can leave a comment below or tweet @PrestonDvorak.
This guide has been updated and maintained based on my own experiences with the encounter, combat log information, and Dungeon Journal information. I'll continue to update it as-needed.
General Mechanics and Abilities
Normal and Heroic
- Dispel Malevolence (magic) in a controlled manner to ensure no players are knocked off the edge or into other harmful abilities.
- On Heroic, avoid the Lingering Malevolence pool the affected player leaves behind.
- At 100% energy, don't stand in front of the boss to avoid Suffering.
- This should be aimed by the tanks and pointed towards Orbs of Torment to remove their Eternal Torment effect, noted below.
- Spawns Orbs of Torment throughout the encounter.
- Orb of Torment
- Immune to damage due to Eternal Torment.
- This effect is removed if the Orb is hit by Suffering.
- Deals light damage over time to random players with Torment.
- After being killed, can be picked up by players and carried to the edge of the platform. From there, the Extra Action Button can be used to throw it off the edge, removing it from the encounter.
- Players carrying an Orb have their movement speed slowed and take light damage over time from Sorrowful Procession.
- Immune to damage due to Eternal Torment.
- Orb of Torment
- Avoid the lines of Grasp of Malice.
- Deals light damage over time to all players throughout the encounter with Aura of Spite.
- At 80%, 60%, and 30% health, Shatter deals light damage to all players.
- Each Shatter increases the damage dealt by Aura of Spite.
- After 80% health, avoid the impact locations of Spite.
Mythic Differences
- At 80%, 60%, and 30% health, Shatter now also drops a piece of armor at Ner'zhul's location which will remain for the rest of the encounter.
- At 80% health, Helm of Suffering drops and will also cast Suffering when Ner'zhul reaches 100% energy.
- At 60% health, Gauntlet of Malice drops and will cause additional Grasp of Malice lines.
- At 30% health, Rattlecage of Agony drops and will cause an additional Malevolence (dispel, magic) debuff.

Healing Notes
Quick Notes, All Difficulties
The notes below mostly pertain to progression difficulty, meaning if you out-gear the difficulty you're on or use extra healers they may not apply.
- Use Major Cooldowns on
- Malevolence (dispel, magic) [occurs about every 30 to 60-seconds]
- Use Minor Cooldowns
- on Shatter
- as-needed to survive the persistent damage from Aura of Spite
- Pre-heal Before (mainly for heal over time classes)
- Potion of Spiritual Clarity use
- in between Malevolence (dispel, magic) and Grasp of Malice
- Other things to watch for
- Healing during the encounter will only get harder as it goes on, so conserving mana early on may be required.
Debuffs to Track
Major
- Malevolence (dispel, magic) should be dispelled in a controlled manner as it knocks back all players.
Minor
- Several abilities cause players to need additional healing over time.
