This guide has been updated and maintained based on my own experiences as well as Dungeon Journal and combat log information. I'll continue to update it as-needed.
Bosses
Below are guides for the bosses in The Necrotic Wake, including their abilities, healer notes, and addon settings.
Blightbone
- The tank will be dealing with Crunch.
- Don't stand in front of the boss when it turns to cast Heaving Retch (dispel, disease) on a random player.
- Spawns Carrion Worms.
- Carrion Worm
- Susceptible to stuns, snares, knock backs, and other crowd control.
- Targets random players, gaining a stack of Blood Gorge with each successful attack, increasing size and damage dealt.
- Five stacks triggers Carrion Eruption, dealing heavy damage over time to all players.
- On Mythic, avoid the Fetid Gas pools left behind upon death.
- Carrion Worm
- Spawns Carrion Worms.
- When Fetid Gas is cast, stay at least 10-yards away from the boss and avoid the resulting Fetid Gas pool left behind.
Healing Notes
- Use cooldowns if Heaving Retch hits multiple players or Carrion Eruption gets out of hand.
Debuffs to Track
- Minor: Heaving Retch (dispel, disease), Carrion Eruption
Addons
Amarth, The Reanimator
- The tank will be dealing with Necrotic Bolt, which can be interrupted when possible.
- Don't stand in front of the boss when Necrotic Breath is cast to avoid the damage and reduced healing received effect.
- On Mythic, players are also affected by Necrotic Ichor, dealing moderate damage over time and further reducing healing received.
- Soothe Unholy Frenzy to remove the increased attack speed effect.
- Land of the Dead summons various undead minions.
- Reanimated Warrior
- The tank will be dealing with Bonerend.
- Reanimated Mage
- Interrupt Frostbolt Volley to prevent the moderate damage dealt to all players and stacking decreased movement speed effect from Chilled (dispel, magic).
- On Mythic, Reanimated Crossbowman
- Deals light damage to random players with Shoot.
- Reanimated Warrior
- When Final Harvest is cast:
- Stay at least 8-yards away from undead minion corpses to avoid the heavy damage.
- Any undead minions that are still alive explode and deal heavy damage to all players.
- On Mythic, Amarth gains a stack of Tortured Echoes for each enemy or player killed by the effects of Final Harvest.
- Tortured Echoes deals increasing damage over time to all players for the remainder of the encounter.
- On Mythic, Amarth gains a stack of Tortured Echoes for each enemy or player killed by the effects of Final Harvest.
Healing Notes
- Use cooldowns as-needed while undead minions are alive.
- Be prepared to use an external defensive cooldown on the tank during Unholy Frenzy if no soothe is available.
Debuffs to Track
- Major: Necrotic Ichor, Necrotic Breath
- Minor: Bonerend, Chilled (dispel, magic)
- Enrages to Soothe: Unholy Frenzy
Addons
Surgeon Stitchflesh
- Avoid the impact locations of Embalming Ichor and the Embalming Ichor pools left behind.
- Targets random players with Stitchneedle, dealing moderate damage and then light damage over time.
- On Mythic, while not at the worktable, Stitchneedle hits all players.
- While at his worktable, Stitchflesh is immune to damage due to Noxious Fog.
- From his worktable he'll Summon Creations.
- Stitchflesh's Creation
- The tank will be dealing with Mutilate.
- Steadily gains stacks of Festering Rot, increasing physical damage dealt and dealing increasing damage over time to all players.
- When targeted by Meat Hook, position so the hook will hit Stitchflesh.
- The spell is cast twice, the first cast sets the direction of the hook, so you can move around freely. The second cast throws the hook, so be sure you move out of the way by then.
- Stitchflesh's Creation
- From his worktable he'll Summon Creations.
- After being pulled from his worktable, Stitchflesh will be vulnerable to damage before he Escapes back to his worktable.
- The tank will be dealing with Sever Flesh.
- When targeted by Morbid Fixation, run away from Stitchflesh.
- This can be interrupted using Stitchflesh's Creation's Meat Hook.
Healing Notes
- Use cooldowns as-needed if the damage from Festering Rot becomes overwhelming.
Debuffs to Track
- Minor: Stitchneedle
Addons
Nalthor the Rimebinder
- The tank will be dealing with Icy Shard.
- Avoid the impact locations of Comet Storm.
- Move at least 16-yards away from the player affected by Frozen Binds (dispel, magic).
- While Icebound Aegis is active, all players take light damage over time.
- On Mythic, taking damage applies a stack of Icebound Aegis, further increasing damage taken.
- At 100% Energy, a player is sent into Dark Exile.
- The affected player has 50-seconds to kill the Zolramus Siphoner and escape before being killed.
- Avoid the impact locations of Comet Storm.
- Avoid the locations of Blizzard.
- Zolramus Siphoner
- Interrupt Enfeeble to prevent the reduced haste and movement speed effect.
- On Mythic, players gain stacks of Frigid Cold, reducing movement speed.
- After returning to the main platform, before the stacks expire, move to the edge of the platform to drop the resulting Razorshard Ice patch that's left behind. This patch will be larger the higher the stacks.
Healing Notes
- Dispel Frozen Binds (magic) when no other players are within 16-yards.
- Use cooldowns while Icebound Aegis is active.

Trash Mobs
Below are notes for the trash mobs in The Necrotic Wake. The Necrotic Wake is a relatively linear dungeon. The trash below is ordered as if you're completing the bosses in the order they're listed above.
Entrance thru Blightbone
- Corpse Harvester
- Interrupt Drain Fluids to prevent the incapacitating effect and light damage over time.
- Deals light damage to random players with Throw Flesh.
- Stitched Vanguard
- Interrupt Meat Shield to prevent the damage absorption effect.
- Soothe Seething Rage (enrage) to remove the increased attack speed effect.
- Zolramus Gatekeeper
- Targets random players with Clinging Darkness (dispel, magic), dealing light damage over time. Dispelling causes the effect to jump to another player.
- Deal light damage to all players with Wrath of Zolramus, the impact locations are technically avoidable, but they impact rapidly.
Blightbone thru Amarth, The Reanimator
- Zolramus Sorcerer
- Avoid the locations of Shadow Wells.
- Zolramus Bonecarver
- The tank will be dealing with Boneflay, reducing maximum health and dealing light damage over time.
- Zolramus Necromancer
- When affected by Grim Fate, move at least 6-yards away from other players.
- Nar'zudah
- Purge Dark Shroud to remove the immunity to physical damage and damage dealt to nearby players effect.
- When affected by Shared Agony, move at least 25-yards from the other linked player to break the effect.
- When affected by Grim Fate, move at least 6-yards away from other players.
- Skeletal Marauder
- Stay at least 10-yards away or interrupt Rasping Scream (dispel, magic) to prevent the Fear effect.
- Don't stand in front of these to avoid Gruesome Cleave.
- Skeletal Monstrosity
- While Reaping Winds is channeled, run against the pull to avoid Chill Scythe when the channel ends.
- Avoid the impact locations of Frigid Spikes.
- Zolramus Bonemender
- Interrupt Bonemend to prevent the healing.
- Heals an enemy with Final Bargain before dealing heavy damage over time.
Amarth, The Reanimator thru Surgeon Stitchflesh
- Spare Parts
- Fixate on random players with Hunt Prey.
- Corpse Collector
- Interrupt Goresplatter (dispel, disease) to prevent the light damage over time effect applied to all players.
- Interrupt Drain Fluids to prevent the incapacitating effect and light damage over time.
- Targets random players with Throw Cleaver, dealing moderate damage to the first player hit.
- Deals light damage to random players with Throw Flesh.
- Flesh Crafter
- Interrupt Repair Flesh to prevent the healing.
- Kyrian Stitchwerk
- Goregrind
- Loyal Creation
- Soothe Vengeful Rage (enrage) to remove the increased attack speed effect.
- When Spine Crush is cast, stay at least 8-yards away to avoid the moderate damage and knock back effect.
- Rotspew
- Deals moderate damage to the tank with Mutilate.
- Targets random players with Spew Disease, leaving a poison patch at their location.
- Stitching Assistant
- Targets random players with Throw Cleaver, dealing moderate damage to the first player hit.
- Deals light damage to random players with Throw Flesh.
- Separation Assistant
- When targeted by Morbid Fixation, run away from the Assistant to avoid taking heavy damage.
- Targets random players with Throw Cleaver, dealing moderate damage to the first player hit.
Surgeon Stitchflesh thru Nalthor the Rimebinder
- There are no trash mobs between these two bosses.
Trash Debuffs to Track
- Major:
- Magic Dispel: Clinging Darkness (dispel, magic), Rasping Scream (dispel, magic)
- Disease Dispel: Goresplatter (dispel, disease)
- Incapacitating: Drain Fluids
- Damaging: Boneflay, Grim Fate, Tenderize, Morbid Fixation
- Minor:
- Damaging: Shared Agony, Gut Slice. (334610)
- Enrages to Soothe: Seething Rage, Vengeful Rage
- Buffs to Purge: Dark Shroud
Trash Addons
Mythic+ Notes
Below are notes that pertain to high mythic+ levels and certain affixes. What constitutes a high mythic+ level will depend on your gear and group, so if you out-gear a particular mythic+ level these notes may not apply. The notes are grouped by affix but are in no particular order.

- Amarth, The Reanimator
- The tank will need significant healing while Unholy Frenzy is active if no enrage dispel is available.
- Surgeon Stitchflesh
- Players handling Meat Hook will take heavy damage due to (326629) .
- Nalthor the Rimebinder
- Icebound Aegis may require cooldowns to survive.
- Amarth, The Reanimator
- Fortified
- Zolramus Gatekeeper
- Wrath of Zolramus may require minor cooldowns.
- Zolramus Bonecarver
- The tank may need external defensive cooldowns at higher stacks of Boneflay.
- Kyrian Stitchwerk / Goregrind
- The tank may need external defensive cooldowns at higher stacks of Tenderize.
- Zolramus Gatekeeper
- Sanguine
- Zolramus Gatekeeper
- Will not move while channeling Wrath of Zolramus, and cannot be knocked back.
- Skeletal Monstrosity
- Will not move while channeling Reaping Winds, and cannot be knocked back.
- The following enemies can be difficult to move due to their preference for casting spells. Be ready with knock backs and interrupts.
