This page covers healer-focused strategies for the Neltharion's Lair dungeon in Legion. While it's tailored for healers, other roles may also find the information useful. If you have any suggestions or feedback, you can leave a comment below or contact me.
This guide has been updated and maintained based on my own experiences as well as Dungeon Journal and combat log information. I'll continue to update it as-needed.
Bosses
Below are guides for the bosses in Neltharion's Lair, including their abilities, healer notes, and addon settings.
Rokmora
- Don't stand in front to avoid Razor Shards.
- Blightshard Skitters spawn throughout the encounter due to Brittle.
- Blightshard Skitter
- Upon death, avoid the Choking Dust cloud left behind.
- Blightshard Skitter
- Deals moderate damage to all players with Shatter.
- Any Blightshard Skitters alive also explode, dealing light damage to all players with Rupturing Skitter.
- Players take light damage over time while moving due to Crystalline Ground.
Mythic Differences
- There are no differences on mythic.
Mythic+ Affix Notes
- No noteable interactions.
Healing Notes
- Use cooldowns as-needed on Shatter.
Debuffs to Track
- There are no particular debuffs to track.
Addons
Ularogg Cragshaper
- The tank will be dealing with extra moderate damage from Sunder.
- Avoid the impact location of Strike of the Mountain, denoted by two stone hands. Players hit take heavy damage.
- During Bellow of the Deeps, kill the Bellowing Idols to stop the Falling Debris.
- Every 60-seconds, during Stance of the Mountain:
- Ularogg transforms into a Bellowing Idol and is shuffled around with other Bellowing Idols.
- When the shuffling ends, players must kill Ularogg's Bellowing Idol to reveal him again.
- Players take increasing damage over time until Ularogg is revealed.
Mythic Differences
- There are no differences on mythic.
Mythic+ Affix Notes
- Can make avoiding Strike of the Mountain more difficult.
Healing Notes
- Use cooldowns on Bellow of the Deeps/Falling Debris.
Debuffs to Track
- There are no particular debuffs to track.
Addons
Naraxas
- If no players are in melee range, Putrid Skies deals heavy damage to all players.
- Every 15-seconds, during Toxic Retch:
- Dispel Toxic Retch (poison) to remove the moderate damage over time effect.
- Avoid the impact locations of Toxic Retch and the pools left behind.
- At 100% Energy, every 50-seconds, during Spiked Tongue:
- If targeted by Spiked Tongue, run against the pull to avoid reaching Naraxas.
- Rancid Maw pools can be used to slow movement speed and help negate the pull effect.
- Every 60-seconds, two Wormspeaker Devout join the encounter.
- Wormspeaker Devout
- Susceptible to Stuns, Snares, and most crowd control effects.
- Attempt to reach to Naraxas to perform Fanatic's Sacrifice, granting her a stack of Ravenous which greatly increases damage dealt.
- Wormspeaker Devout
- Avoid the impact locations of Rancid Maw.
- Deals increasing light damage over time to all players with Hurling Rocks.
- At 20% health, goes into a Frenzy, greatly increasing Attack Speed and Physical damage dealt.

Healing Notes
- Use cooldowns as-needed on Toxic Retch, especially while Frenzy is active.
Debuffs to Track
- Major: Spiked Tongue, Devouring
- Minor: Toxic Retch (dispel, poison)
Addons
Dargrul the Underking
- The tank will be dealing with extra moderate damage from Molten Crash.
- Don't stand in front to avoid Landslide, targeted at the tank. Players hit take heavy damage.
- Additionally, this destroys any Crystal Spikes it hits.
- Avoid the impact location of Crystal Spikes. Players hit take moderate damage and are knocked back.
- This leaves behind a Crystal Spike, used to deal with Magma Wave, detailed below.
- Every 60-seconds, during Magma Wave:
- Break line of sight on Dargrul using a Crystal Spike to avoid the heavy damage over time from Magma Wave.
- Avoid the cracks in the ground that spew Lava Geyser. Players in them take light damage over time.
- Avoid the impact location of Flame Gout and the pool left behind. Players hit take moderate damage and players in the area take moderate damage over time.
- Every 75-seconds, summons a Molten Charskin with Magma Sculptor.
- Molten Charskin
- Susceptible to Stuns, Snares, and most crowd control effects.
- Fixates on a random player with Burning Hatred.
- Can be led over a Crystal Spike to be Stunned by Crystal Cracked, greatly increasing damage taken.
- While moving, deals moderate damage over time to all players with Magma Breaker.
- Molten Charskin
- While moving, deals moderate damage over time to all players with Magma Breaker.
Mythic Differences
- There are no differences on mythic.
Mythic+ Affix Notes
- Can make positioning for Magma Wave more difficult.
Healing Notes
- Use cooldowns on Magma Wave.
Debuffs to Track
- Major: Burning Hatred

Addons
Trash Mobs
Below are notes for the trash mobs in Neltharion's Lair. Neltharion's Lair has a relatively linear layout, and trash mobs are ordered as if you're completing bosses in the order they're listed above.
Entrance thru Rokmora
- Vileshard Crawler
- Upon death, avoid the Acid Splatter pool left behind. Players in the area take light damage over time.
- Embershard Scorpion
- Deals light damage over time to random players with Impaling Shard.
- Rockback Gnasher
- The tank will be dealing with extra moderate damage and light damage over time from Bone Chomp.
- Interrupt Stone Gaze to prevent the stun effect.
- Soothe immediately.
- Tarspitter Lurker
- Submerges and reappears near a random player.
- Don't stand in front to avoid Viscid Bile.
- Upon death, avoid the Rancid Ooze pool left behind. Players in the area take light damage over time.
- Will not move while channeling Viscid Bile.
- Submerges and reappears near a random player.
- Vileshard Hulk
- Don't stand in front to avoid Piercing Shards. Players hit take heavy damage and moderate damage over time.
- Stay more than 15-yards away from the tank to avoid Fracture. Players hit take moderate damage and are knocked back.
- Should be killed first or otherwise prevented from gaining excessive stacks.
- Vileshard Chunk
- Stay more than 10-yards away to avoid Crush. Players hit take moderate damage.
Rokmora thru Ularogg Cragshaper
- This area includes Rockback Gnashers, detailed above.
- To begin this section, players must click on an Empty Barrel to ride down the waterfall.
- Rockbound Pelter
- Deals moderate damage to random players with Jagged Disc.
- Can leap away with Retreat.
- Should be killed first or otherwise prevented from gaining excessive stacks.
- Soothe immediately.
- Can be difficult to move due to their preference for casting spells.
- Mightstone Breaker
- Stoneclaw Hunter
- Stay more than 5-yards away to avoid Stone Shatter.
- Understone Drummer
- Runs to a nearby Drums of War and attempts to summon additional enemies with War Drum.
- Drums of War
- Can be killed to prevent Understone Drummer from using War Drum.
- Blightshard Shaper
- Interrupt Stone Bolt to prevent the moderate damage dealt to the tank.
- Spawns a Petrifying Totem with Petrifying Totem.
- Petrifying Totem
- Stay more than 5-yards away to avoid Petrified.
- Petrifying Totem
- Can be difficult to move due to their preference for casting spells.
- Burning Geode
- Avoid the impact locations of Scorch. Players hit take heavy damage.
- Can be difficult to move due to their preference for casting spells.
Ularogg Cragshaper thru Naraxas
- This area includes Blightshard Shapers, Rockbound Pelters, Mightstone Breakers, Rockback Gnashers, and Tarspitter Lurkers, detailed above.
- Stoneclaw Grubmaster
- Stay more than 5-yards away to avoid Stone Shatter.
- Spawns a Tarspitter Grub with Worm Call.

- Must be killed, Stunned, or otherwise incapacitated before the Metamorphosis cast finishes, transforming it into a Rotdroll Grabber.
- Rotdrool Grabber
- Avoid the Rancid Pool created when spawned. Players in the area take light damage over time.
- Pulls random players to melee range with Barbed Tongue.
- Rotdrool Grabber
- Must be killed, Stunned, or otherwise incapacitated before the Metamorphosis cast finishes, transforming it into a Rotdroll Grabber.
- Tarspitter Grub
- Deals light damage over time to random players with Leech.
Naraxas thru Dargrul
- This area includes Mightstone Breakers, detailed above.
- Understone Demolisher
- Transforms into a Colossal Charskin with Charskin.
- Colossal Charskin
- Fixates on a random player with Burning Hatred.
- Colossal Charskin
- Transforms into a Colossal Charskin with Charskin.
- Rockbound Trapper
- Interrupt Bound to prevent the Stun and light damage over time effect on a random player.
- Deals moderate damage to random players with Jagged Disc.
- Should be killed first or otherwise prevented from gaining excessive stacks.
- Soothe immediately.
- Can be difficult to move due to their preference for casting spells.
- Emberhusk Dominator
- Don't stand in front to avoid Ember Swipe.
- Soothe Frenzy to remove the increased Attack Speed and damage dealt effect.
- Deals moderate damage to up to three random players with Crystal Spike.
- Should be killed first or otherwise prevented from gaining excessive stacks.
Trash Debuffs to Track
