This guide has been updated and maintained based on my own experiences as well as Dungeon Journal and combat log information. I'll continue to update it as-needed.
Bosses
Below are guides for the bosses in Neltharion's Lair, including their abilities, healer notes, and addon settings.
Rokmora
- Don't stand in front to avoid Razor Shards.
- During Crystalline Ground, avoid moving as much as possible to avoid taking light damage over time.
- Brittle causes Blightshard Skitters to spawn throughout the encounter.
- Blightshard Skitter
- Upon death, avoid the Choking Dust clouds left behind.
- Blightshard Skitter
- Deals moderate damage to all players with Shatter.
- Any Blightshard Skitters alive also explode, dealing light damage to all players with Rupturing Skitter.
Healing Notes
- Use cooldowns on Shatter, especially if there are Blightshard Skitters alive.
Debuffs to Track
- There are no particular debuffs to track.
Addons
Ularogg Cragshaper
- The tank will be dealing with Sunder.
- Don't stand between the sets of Strike of the Mountain hands.
- During Bellow of the Deeps, kill the Bellowing Idols to stop the Falling Debris.
- During Stance of the Mountain:
- Ularogg transforms into a Bellowing Idol and is shuffled around with other Bellowing Idols.
- When the shuffling ends, players must kill Ularogg's Bellowing Idol to reveal him again.
- Players take increasing damage over time until Ularogg is revealed.
Healing Notes
- Use cooldowns during Stance of the Mountain.
Debuffs to Track
- There are no particular debuffs to track.
Addons
Naraxas
- If no players are in melee range, Putrid Skies deals heavy damage to all players.
- When targeted by Rancid Maw, move more than 5-yards away from other players to drop the resulting pool.
- Avoid the impact locations of Toxic Retch (dispel, poison) and the resulting pools.
- Wormspeaker Devout jump in from the stands.

- Susceptible to slows, snares, and other crowd control effects.
- Attempt to run to Naraxas, granting her a stack of Ravenous with Fanatic's Sacrifice
- Wormspeaker Devout
- Players take increasing light damage over time from Hurling Rocks
- At 100% energy, if affected by Spiked Tongue, run against the pull to avoid Devouring and granting Naraxas a stack of Ravenous.
- Rancid Maw pools can be used to further slow your movement speed against the pull.
- At 20% health, goes into a Frenzy, greatly increasing attack speed and physical damage dealt.
Healing Notes
- The player affected by Spiked Tongue may need external defensive cooldowns.
Debuffs to Track
- Major: Spiked Tongue, Devouring
- Minor: Toxic Retch (dispel, poison)
Addons
Dargrul
- Don't stand in front when he turns to cast Landslide.
- Immediately following, the tank will be dealing with Molten Crash.
- Avoid the impact locations of Crystal Spikes.
- Magma Sculptor summons a Molten Charskin.
- Molten Charskin
- Fixates on a random player with Burning Hatred.
- Should be led to a Crystal Spike to be stunned by Crystal Cracked, greatly increasing damage taken.
- While moving, deals light damage to all players with Magma Breaker.
- Steadily gains stacks of Rising Inferno, increasing movement speed and damage dealt.
- Fixates on a random player with Burning Hatred.
- Molten Charskin
- During Magma Wave, break line of sight on Dargrul using a Crystal Spike to avoid the heavy damage over time.
- Players take light damage over time even when behind a Crystal Spike.
- Avoid the cracks in the ground that spew Lava Geyser.
- Avoid the impact locations of Flame Gout.
- While moving, deals light damage to all players with Magma Breaker.
Healing Notes
- Use cooldowns during Magma Wave.
- The player targeted by Burning Hatred may need an external defensive cooldown if they're going to be hit by the Molten Charskin.

Trash Mobs
Below are notes for the trash mobs in Neltharion's Lair. Neltharion's Lair has a relatively linear layout. The trash below is ordered as if you're completing the bosses in the order they're listed above.
Entrance thru Rokmora
- Vileshard Crawler
- Upon death, avoid the Acid Splatter pool.
- Embershard Scorpion
- Deals light damage over time to random players with Impaling Shard.
- Rockback Gnasher
- The tank will be dealing with Bone Chomp, dealing light damage over time.
- Interrupt Stone Gaze to prevent the stun effect.
- Tarspitter Lurker
- Submerge and reappear near a player, don't stand in front of it when it casts Viscid Bile.
- Upon death, avoid the Rancid Ooze pools.
- Vileshard Hulk
- Don't stand in front to avoid Piercing Shards and Fracture.
Rokmora thru Ularogg Cragshaper
- This area includes Rockback Gnashers, detailed above.
- To begin this section, players must click on an Empty Barrel to ride down the waterfall.
- Rockbound Pelter
- Deals moderate damage to random players with Jagged Disc.
- Will leap away with Retreat.
- Mightstone Breaker
- Don't stand in front to avoid Avalanche, also avoid the impact locations targeted at players.
- Stoneclaw Hunter
- Stay more than 10-yards away to avoid Stone Shatter.
- Understone Drummer
- Runs to a nearby Drums of War and attempts to summon additional enemies with War Drum.
- Drums of War
- Can be killed to prevent Understone Drummer from using War Drum.
- Blightshard Shaper
- Interrupt Stone Bolt to prevent the moderate damage dealt to the tank.
- Kill the Petrifying Totem or stay more than 5-yards away to avoid being Petrified.
- Burning Geode
- Avoid the impact locations of Scorch.
Ularogg Cragshaper thru Naraxas
- This area includes Blightshard Shapers, Rockbound Pelters, Mightstone Breakers, Rockback Gnashers, and Tarspitter Lurkers, detailed above.
- Stoneclaw Grubmaster
- Stay more than 10-yards away to avoid Stone Shatter.
- Spawns a Tarspitter Grub with Worm Call.

- Must be killed, stunned, or incapacitated before the Metamorphosis cast finishes and it turns into a Tarspitter Lurker.
- Tarspitter Grub
- Deals light damage over time to random players with Leech.
Naraxas thru Dargrul
- This area includes Mightstone Breakers, detailed above.
- Understone Demolisher
- Transforms into a Colossal Charskin with Charskin.
- Colossal Charskin
- Fixates on a random player with Burning Hatred.
- Colossal Charskin
- Transforms into a Colossal Charskin with Charskin.
- Rockbound Trapper
- Interrupt Bound to prevent the stun effect.
- Deals moderate damage to random players with Jagged Disc.
- Emberhusk Dominator
- Don't stand in front to avoid Ember Swipe.
- Soothe Frenzy to remove the increased attack speed and damage dealt effect.
- Deals moderate damage to up to three random players with Crystal Spike
Trash Debuffs to Track

Mythic+ Notes
Below are notes that pertain to high mythic+ levels and certain affixes. What constitutes a high mythic+ level will depend on your gear and group, so if you out-gear a particular mythic+ level these notes may not apply. The notes are grouped by affix but are in no particular order.
