Echo of Neltharion, Healer Guide
Updated 2 weeks, 6 days ago for patch 10.1 by Preston. View the change log.
This page covers a healer-focused strategy for Echo of Neltharion in Aberrus, the Shadowed Crucible in Dragonflight. While it's tailored for healers, other roles may also find the information useful. If you have any suggestions or feedback, you can leave a comment below or contact me.
This guide has been updated and maintained based on my own experiences with the encounter, combat log information, and Dungeon Journal information. I'll continue to update it as-needed.
General Mechanics and Abilities
Normal and Heroic
Stage One - The Earth-Warder
This stage begins the encounter and ends when Neltharion reaches 70% health.
- The tanks will be dealing with extra light damage and the knockback from Calamitous Strike, as well as the greatly increased Physical damage taken effect from Calamitous Strike.
- When targeted by Volcanic Heartbeat, stay more than 10-yards away from other players to avoid chaining the heavy damage and moderate damage over time effect from Volcanic Heart when the effect expires.
- Every 30-seconds, during Echoing Fissure:
- Avoid the large impact location under Neltharion's current location, as well as any previous Echoing Fissure impact locations, to avoid the fatal damage dealt.
- Avoid the impact locations of Collapsed Earth (dispel, magic), targeted at random locations. Players hit take light damage and are Stunned.
- Avoid the growing Seeping Lava pools at previous Echoing Fissure locations.
- Deals light damage to all players with Echoing Fissure.
- During Rushing Darkness:
- If targeted by Rushing Darkness position to not be knocked into Twisted Earth segments.
- Twisted Earth segments hit by a knocked back player Shatter, dealing moderate damage to the player, and dealing stacking light damage over time to all players with Shatter.
- Avoid the impact locations of Shattered Rock. Players hit take light damage.
- Twisted Earth segments hit by a knocked back player Shatter, dealing moderate damage to the player, and dealing stacking light damage over time to all players with Shatter.

- About every 30-seconds, during Twisted Earth, avoid the impact locations of Twisted Earth segments. Players hit take moderate damage and are knocked back from Volcanic Blast.
- Twisted Earth segments break line of sight and cannot be passed through, making it important for players to not be split up.
Intermission One - Giving in to Darkness
This intermission begins when Neltharion reaches 70% health and ends after about 10-seconds, when Neltharion's Surrender to Corruption finishes channeling.
- To begin the intermission, stay more than 5-yards away from any remaining Twisted Earth segments to avoid Volcanic Blast. Players hit take moderate damage and are knocked back.
- Additionally, avoid the impact locations of Collapsed Earth (dispel, magic), targeted at random locations. Players hit take light damage and are Stunned.
- Deals light damage over time to all players with Surrender to Corruption, and moderate damage to all players when the channel ends.
- On Heroic, when the channel ends, several players are affected by Surrendering to Corruption, and then Corruption. Making them hostile to all other players and allowing them to see and attack enemies affected by Hidden in Void, detailed below.
Stage Two - Corruption Takes Hold
This stage begins when Neltharion's Surrender to Corruption finishes channeling in intermission one and ends when all Voices from Beyond are killed.
- Three Voices from Beyond spawn to begin the stage.
- Voice from Beyond
- Initially affected by Hidden in Void, a large damage absorption shield. On Heroic, this makes it visible only to players affected by Corruption until the shield is depleted.
- Once the shield is depleted, deals light damage over time to all players with Ruptured Veil until killed.
- While alive, grants Neltharion Empower Shadows, allowing him to cast Shadow Barrier at 40% health, making him effectively immune to damage.

- On Heroic, continually gains stacks of Unstable Mutation, greatly increasing damage dealt.

- Every 30-seconds, during Ebon Destruction:
- Stand within 5-yards of a void portal to gain Sundered Reality and prevent the fatal damage dealt.
- This deals light damage and then light damage over time to all players.
Mythic Differences
All Stages
- Breaking a Twisted Earth segment applies Shatter to the player, greatly increasing damage taken from breaking additional Twisted Earth segments while the effect persists.
- Volcanic Heartbeat now affects all players in line of sight of Neltharion.
Stage Three - Reality Fractures
- Void portals are destroyed when Sundered Reality protects one or more players from Ebon Destruction.
Healing Notes
Quick Notes, All Difficulties
The notes below mostly pertain to progression difficulty, meaning if you out-gear the difficulty you're on or use extra healers they may not apply.
- Use Major Cooldowns
- stage one: Echoing Fissure [occurs every 30-seconds]
- stage two: Umbral Annihilation [occurs every 30-seconds]
- stage three: Ebon Destruction [occurs every 30-seconds]
- Use Minor Cooldowns
- as-needed when Twisted Earth segments Shatter
- as-needed to supplement major cooldowns for abilities noted above
- Pre-heal Before (mainly for heal over time classes)
- Potion of Frozen Focus use
- immediately as stage three begins
- Other things to watch for
- Healing required stays pretty consistently high throughout the encounter, with a roughly 30-second lull between the end of stage two and the first Ebon Destruction.
Debuffs to Track
Major
- Collapsed Earth (dispel, magic) should be dispelled immediately.
- Several abilities cause players to need additional healing.
