Drest'agath, Healer Guide

This guide has been updated and maintained based on my own experiences with the encounter, logs information, and Dungeon Journal information. I'll continue to update it as-needed.
General Mechanics and Abilities
Normal and Heroic
During this encounter players will be killing Drest'agath's Appendages to gain a buff allowing permanent damage to be dealt to Drest'agath.
- Drest'agath
- Rapidly heals damage taken due to Aberrant Regeneration.
- Killing an Appendage of Drest'agath (detailed below) leaves behind a pool of Void Infused Ichor.
- Touching the pool allows the affected player to damage Drest'agath while the effect persists, avoiding its healing effect.
- On Heroic, when the effect expires the player gains Umbral Aversion and cannot be affected by Void Infused Ichor for 60-seconds.
- Killing an Appendage of Drest'agath (detailed below) leaves behind a pool of Void Infused Ichor.
- The tanks will be dealing with Volatile Seed.
- Upon expiring, Volatile Detonation applies Volatile Corruption to all players and Appendages within 15-yards, increasing damage taken.
- If no players are in melee range, Void Grip stuns and pulls in the nearest player.
- At 100% Energy, Throes of Agony deals heavy damage over time to all players.
- Killing an Appendage of Drest'agath gives 25 Energy over 5-seconds.
- When affected by Mutterings of Insanity, move at least 15-yards away from other players before becoming immobile and triggering Unleashed Insanity.
- During Entropic Crash, move as far away from Drest'agath and Tentacles of Drest'agath as possible to reduce damage taken.
- Avoid the Void Glare beams from Drest'agath and Eyes of Drest'agath.
- Rapidly heals damage taken due to Aberrant Regeneration.
- Appendages of Drest'agath
- Each Appendage has an ability they will use when Drest'agath casts Throes of Agony, noted under each Appendage below.
- When killed, Appendages:
- Leave behind a pool of Void Infused Ichor.
- Grant Drest'agath 25 Energy over 5-seconds from Dismemberment.
- Eye of Drest'agath
- Deals light damage over time to random players with Mind Flay.
- During Throes of Agony, avoid the impact locations of Falling Gore, triggered by Errant Blast.
- Tentacle of Drest'agath
- Avoid the location of Crushing Slam.
- During Throes of Agony, avoid the Reality Tear as it moves around the room.
- On Heroic, Void Miasma blocks line of sight into and out of a 10-yard area around the Tentacle.
- Maw of Drest'agath
- Deal light damage to random players with Acid Splash.
- During Throes of Agony, avoid the Spine Eruption rings that appear around the Maw.
- On Heroic, Mutterings of Betrayal places a stack of a healing absorption effect on any players within 10-yards.
- This effect must be healed off, at 4-stacks the player becomes hostile due to Betrayed.
Mythic Differences
- Appendages of Drest'agath
- Upon death, now also trigger the 100% Energy ability on all other Appendages of the same type due to Throes of Dismemberment.
Healing Notes
Quick Notes, All Difficulties
The notes below mostly pertain to progression difficulty, meaning if you out-gear the difficulty you're on or use extra healers they may not apply.
- Use Major Cooldowns on
- Use Minor Cooldowns
- Pre-heal Before (mainly for heal over time classes)
- Potion of Replenishment use
- likely after Throes of Agony when the overall Appendage count is low
- Other things to watch for
- Be aware of players inside Void Miasma as you may not have line of sight on them.
Debuffs to Track
Major
- Volatile Corruption increases the damage players take.
- Mutterings of Betrayal's healing absorption effect must be healed off quickly.
Minor
- Mind Flay causes players to need additional healing.
DBM Settings
Below are the DBM settings I'll be using for Drest'agath. You can set them up however you'd like, but these ones should work well as a starting point. Some settings may be Mythic only.
