This page covers healer-focused strategies for The Underrot dungeon in Battle for Azeroth. While it's tailored for healers, other roles may also find the information useful. If you have any suggestions or feedback, you can leave a comment below or contact me.
This guide has been updated and maintained based on my own experiences as well as Dungeon Journal and combat log information. I'll continue to update it as-needed.
Bosses
Below are guides for the bosses in The Underrot, including their abilities, healer notes, and addon settings.
Elder Leaxa
- Players hit by any ability gain a stack of Taint of G'huun, dealing stacking light damage over time and absorbing a moderate amount of healing.
- Interrupt Blood Bolt to prevent the light damage dealt to the tank.
- Don't stand in front when she turns to cast Creeping Rot. Players hit take light damage over time from Creeping Rot.
- Every 45-seconds, during Blood Mirror, a Blood Effigy spawns at a random location on the platform.
- Blood Effigy
- Uses the same abilities as Leaxa.
- Blood Effigy
- On Heroic, every 30-seconds, avoid the Sanguine Feast pool where Lexaxa teleports to, targeted at a random player's location.
Mythic Differences
- There are no differences on mythic.
Mythic+ Affix Notes
Can make avoiding Creeping Rot and Sanguine Feast more difficult.
Healing Notes
- Use cooldowns as-needed to heal with high stacks of Taint of G'huun.
Debuffs to Track
- Major: Taint of G'huun
- Minor: Creeping Rot
Addons
Cragmaw the Infested
- Infested causes abilities to spawn Blood Tick Larva.
- Blood Tick Larva
- Killed by walking over it.
- Attempts to cast Metamorphosis, transforming into a Blood Tick if the cast finishes.
- Blood Tick
- The tank will be dealing with stacks of Serrated Fangs, dealing stacking light damage over time.
- Triggers Blood Burst on death, dealing light damage to all players.

- Blood Tick Larva
- Don't stand in front to avoid Indigestion, targeted on the tank. Players hit take heavy damage.
- Don't stand in front when it turns to cast Charge, targeted at a random player's location. Players hit take heavy damage and are knocked back.
- On Heroic, at 100% Energy, every 45-seconds, deals moderate damage over time to all players with Tantrum.
Mythic Differences
- There are no differences on mythic.
Mythic+ Affix Notes
Healing Notes
- Use cooldowns on Tantrum.

- Major: Serrated Fangs
Addons
Sporecaller Zancha
- Don't stand in front to avoid Shockwave, targeted on the tank. Players hit take heavy damage.
- Additionally, any Volatile Pods (detailed below hit are destroyed.
- When targeted by Upheaval, stay more than 8-yards away from other players to avoid chaining the moderate damage dealt and knock up effect from Upheaval.
- Additionally, any Volatile Pods (detailed below) within 8-yards of affected players are destroyed.
- Every 45-seconds, during Boundless Rot:
- Avoid the locations of Volatile Pods spawned around the area.
- Player who touch a Volatile Pod take moderate damage from Decaying Spores and gain a stack of Decaying Spores (dispel, disease).
- Volatile Pods are destroyed when hit by Upheaval or Shockwave.
- At 100% Energy, every 45-seconds, deals moderate damage to all players with Festering Harvest.
- Additionally, all players gain one stack of Decaying Spores (dispel, disease) for each Volatile Pod that hasn't been destroyed.
- On Heroic, avoid the paths of Volatile Pods orbs are they travel across the area. Players hit take light damage and gain a stack of Decaying Spores (dispel, disease).
Mythic Differences
- There are no differences on mythic.
Mythic+ Affix Notes
Can make positioning for Upheaval more difficult.
Healing Notes
- Use cooldowns on Festering Harvest or to deal with high stacks of Decaying Spores (dispel, disease).
Debuffs to Track
- Major: Decaying Spores (dispel, disease), Upheaval
Addons
Unbound Abomination
- Blood Barrier converts all damage taken into Blood Energy.
- At 100% Blood Energy, two Blood Visages spawn in the room.
- Blood Visage
- When killed, Fatal Link damages the Unbound Abomination for 17% of its maximum health.

- At 100% Blood Energy, two Blood Visages spawn in the room.
- Avoid the impact locations of Vile Expulsion and the Vile Expulsion pools left behind. Players in the impact take heavy damage, players remaining in the area take light damage over time.
- Rotting Spores spawn steadily throughout the encounter.

- Deals moderate damage to any player it touches and creates a Vile Expulsion pool.
- Rotting Spore
- Players are assisted by Titan Keeper Hezrel.
- Titan Keeper Hezrel
- Stack with the player targeted by Cleansing Light to remove all stacks of Putrid Blood (dispel, magic).
- Additionally, this removes all Vile Expulsion pools it hits.
- Deals heavy damage over time to and Stuns Blood Visages with Purge Corruption.
- Stack with the player targeted by Cleansing Light to remove all stacks of Putrid Blood (dispel, magic).
- Titan Keeper Hezrel
- Deals stacking light to moderate damage over time to all players throughout the encounter with Putrid Blood (dispel, magic).
- In addition to being dispelled, stacks are removed from all players in the effect of Cleansing Light, detailed above.
Mythic Differences
- There are no differences on mythic.
Mythic+ Affix Notes
Can make stacking for Cleansing Light more difficult.
Healing Notes
- Use cooldowns as-needed to deal with high stacks of Putrid Blood (dispel, magic).

Addons
Trash Mobs
Below are notes for the trash mobs in The Underrot. The Underrot has a relatively linear layout, and trash mobs are ordered as if you're completing bosses in the order they're listed above.
Entrance thru Elder Leaxa
- Fetid Maggot
- Don't stand in front to avoid Rotten Bile. Players in the area take moderate damage over time.
- Underrot Tick
- Deals light damage over time to the tank with Blood Maw.
- Upon death, deals light damage to all players with Blood Burst.
- Fortified Cooldowns may be needed as these die and trigger Blood Burst.
- Chosen Blood Matron
- Charges random players, dealing light damage with with Blood Harvest.
- Upon reaching the player, don't stand in front to avoid Savage Cleave. Players hit take light damage over time.
- Soothe Warcry to remove the increased Attack Speed effect.
- Charges random players, dealing light damage with with Blood Harvest.
- Fanatical Headhunter
- Targets random players with Hooked Snare, Rooting and dealing light damage over time.
- Deals light damage to random players with Barbed Spear.
- Devout Blood Priest
- Interrupt or purge Gift of G'huun to prevent the unkillable buff.
- Interrupt Dark Reconstitution to prevent the healing.
- When possible, interrupt Blood Bolt to prevent the light damage dealt to the tank.
Soothe immediately.
Can be difficult to move due to their preference for casting spells.
- Befouled Spirit
Elder Leaxa thru Cragmaw the Infested
- This area includes Fetid Maggots, detailed above.
- Living Rot
- Avoid the Foul Sludge pools left behind by Wave of Decay. Players in the area take moderate damage over time.
- Feral Bloodswarmer
- Interrupt Sonic Screech to prevent the interrupt effect and light damage dealt to all players.
- Fixates on random players with Thirst For Blood.
Soothe immediately.
- Diseased Lasher
- Stun to interrupt or dispel Decaying Mind (disease) to remove the Stun and healing absorption effect.
- Healing the player to consume the healing absorption also removes the effect.
Soothe immediately.
- Stun to interrupt or dispel Decaying Mind (disease) to remove the Stun and healing absorption effect.
Cragmaw the Infested thru Sporecaller Zancha
- Fallen Deathspeaker
- Interrupt Raise Dead to prevent a Reanimated Guardian from being summoned.
- Interrupt or soothe Wicked Frenzy to prevent the increased Haste and movement speed.
- Dispel Wicked Embrace (magic) to remove the light damage over time effect.
Soothe immediately.
Can be difficult to move due to their preference for casting spells.
- Reanimated Guardian
- Interrupt or purge Bone Shield to prevent the damage absorption effect.
- Grotesque Horror
- Interrupt Dark Echoes to prevent the light damage dealt to all players.
- When possible, interrupt Death Bolt to prevent the light damage dealt to a random player.
Soothe immediately.
Does not move at all.
- Bloodsworn Defiler
- Interrupt Withering Curse to prevent the stacking increased damage taken and reduced damage dealt effect applied to all players.
- When possible, interrupt Shadow Bolt Volley to prevent the moderate damage dealt to all players.
- Stay more than 6-yards away from the Spirit Drain Totem to avoid Spirit Drain (dispel, magic). Players hit take heavy damage and are Stunned.
Can be difficult to move due to their preference for casting spells.
Sporecaller Zancha thru Unbound Abomination
- This area includes Grotesque Horror, Bloodsworn Defiler, Reanimated Guardian, and Fallen Deathspeaker, as detailed above.
- Faceless Corruptor
- Don't be in front of these it turns to cast Maddening Gaze (dispel, magic). Players hit take moderate damage and are Feared.
- Don't stand in front of the tentacles created by Abyssal Reach. Players hit take moderate damage and are knocked back.
Can make avoiding Maddening Gaze and Abyssal Reach more difficult.
Trash Debuffs to Track
- Major:
- Magic Dispel: Spirit Drain (dispel, magic), Wicked Embrace (dispel, magic), Maddening Gaze (dispel, magic)
- Disease Dispel: Decaying Mind (dispel, disease)
- Damaging: Savage Cleave, Thirst For Blood, Withering Curse, Dark Omen
- Minor:
- Damaging: Blood Maw, Hooked Snare, Dark Echoes
- Enrages to Soothe: Warcry
- Buffs to Purge: Bone Shield, Gift of G'huun
- Root Effects: Hooked Snare
